Naval Vessel Launch Goes Wrong, Ends in Serious Damage

He said a mistake so grave was inexcusable and should never have happened.
No reports of injury or death have been released since the incident. The emphasis is on repairing the warship and understanding the failings here that should not have led to a launch failure.
